Consent Form

New York Mills Union Free School District will host a vaccination clinic in the Elementary Gymnasium for students ages 5-11 on Tuesday, Jan. 4, 2022, from 4-6 p.m.

Provided will be the COVID-19 Pfizer vaccination.

Appointments are reserved on a first-come, first-serve basis online as per the request of Oneida County due to the level of interest for this vaccine.

A consent form will need to be completed for each child getting vaccinated.

It’ll save time if families fill out and bring the consent form to the appointment, but it is not necessary, since it can be done at the appointment.
